Physical Therapist

(illustrative only) Makes home visits to evaluate the degree of function and disability of patients and home environment for safety; Performs physical therapy procedures upon the written order or referral of a licensed physician; Instructs Home Health Aides in their performance of Physical Therapy related tasks such as transfer techniques, home exercise programs; Establishes treatment plan for patients; Participates with other personnel in the formulation of patient's total care plans; Instructs patients and their families on physical therapy techniques and safety recommendations; Confers periodically with a superior and physicians regarding all facets of patient care and related activities; Records on patients' charts all physical therapy evaluations, reevaluations, progress notes, consultation and discharge summaries; Coordinates the provision of required services and supplies with health care vendors; Participates in committee meetings and educational programs related to rehabilitation services.

Candidates must meet the following requirements on or before the
date of application:
Licensed and currently registered by the New York State Education Department as a Physical Therapist. ALL CANDIDATES MUST SUBMIT A COPY OF THEIR COLLEGE TRANSCRIPT AND LICENSE
AT TIME OF APPLICATION.
NOTE: Finger printing may be required for this position. Any fee will be paid by the applicant.

For this position, no written test is required. Candidates will be rated on an evaluation of their training and experience in relation to the duties and requirements of the position. It is essential that candidates give complete and accurate information regarding their training and experience on their applications because one's place on the eligible list will be determined by the rating given in the evaluation. NOTE: For training and experience rating of applications, candidates must have a license to practice as a Physical Therapist in New York State at the time of filing application.
THE PASSING GRADE FOR THIS EXAMINATION IS 70.0.
